We read the article “A preliminary study of the surgical approach for posterior tibial
plateau fractures: Based on posterior fragment segment classification” by Zhu et al.,
with great interest [

]. The article has explained the proposed posterior fragment classification and surgical
approaches that can be used for various combinations of posterior fractures, based
on the region involved. However, there are a few points that need detailed explanation
by the authors for better understanding.
- 1.The authors have remarked on the various surgical approaches used for different posterior column fracture patterns. If the authors could elaborate on how they approached cases with concomitant anterior segment proximal tibia fractures, and procedural tips about the positioning of the patient during the surgery, that would enhance the learning of the readers for the same.
- 2.In conclusion of the article, the authors state that an optimal surgical approach can be predicted using the posterior fragment segment classification only from X-rays. However, we feel that X-ray imaging makes identifying different posterior segment fractures difficult. Additionally, we believe, the column can be better classified using a CT scan as the modality of choice as shown by the authors in their representative cases.
